How to Make a Referral (GHC)

Out-Patient Referrals for GHC Patients

A GHC Outside Service Request referral will need to be created within the GHC system by a GHC staff member.

1. Once CPO and /or the patient has the GHC referral, then an appointment can be scheduled. CPO will call on all GHC referrals received and patient can also call in and schedule.

(For a complete order we would need: patient name, patient phone number, DOB, referring doctor, diagnosis, device requested and the patient insurance carrier(s) and ID #’s)

2. Patients will need their GHC card(s) in hand when they arrive for appointment at CPO.

3. CPO will request authorization from GHC for covered devices.

CPO's clinical team consists of a team of practitioners with a combined total of over 92 years of clinical experience. Each practitioner is both board certified and state licensed in prosthetics as well as in orthotics.
